Parents,

CHSAA Sports Physicals are good for one full calendar year.  Please call Eads Medical Clinic this summer to set up an appointment for your student(s) sports physical for the next school year.  The cost will be $20.  Additionally, your student(s) will receive a coupon redeemable for a free 20 minute movement and injury screen, performed by the hospital physical therapy department.  This screen is entirely optional and will not function to keep athletes out of practice or competition, but is intended to address any nagging aches, pains, or tightness that may be preventing your student from performing their best and/or increasing their risk for injury during the season.  Please call 719-438-2251 prior to August 1st to make an appointment and please call the hospital at 438-5401 to schedule the 20 minute screen.  Thank you.

(Students are also welcome to receive his or her physical from any other medical clinic such as Wiley or Lamar (336-6767), however please note a CHSAA physical form is required.  Thank you)

*Reminder, physicals must be handed into the office or coaches the first week of school.  Physical forms can be picked up and dropped off at Ms. Jackie, Mrs. Cole, Mrs. Britten, or Mr. Bohlander’s office.  Please call Kiowa County School District RE-1 at719-438-2216 if you have any questions.  Thank you
